Planning the Next Steps: Are you wondering what comes after high school? We've got you covered! Gain a comprehensive understanding of the transition planning process, from exploring day programs, college, and continuing education to carving out a career path and understanding guardianship options.
This fair is a one-stop destination to discover the wealth of opportunities and support available as you embark on this exciting journey.
Community organizations will be available to help families learn about opportunities and steps involved with the process of planning for life after high school, including:

CBS Transition Planning Services
While at the fair, families can choose to register for CBS’s transition planning services. The program, for students with disabilities aged 12 to 21, offers families a streamlined process of learning about transition planning, community resources, OPWDD eligibility, and supports available after graduation.
Transition planning can be overwhelming, and many families don’t know what options are available for their children or what services they will be eligible to receive. The CBS transition coordinator assists students and caregivers in navigating the systems of care to meet transition goals. The program aims to increase awareness of options for career and vocational pursuits, higher education aspirations, day programs, and resources available to individuals with disabilities in Dutchess County, making sure every student has the supports they want and need to reach their goals and achieve their dreams.

Dutchess County Agency Partner Grant
The Transition Fair is paid for in part by Dutchess County through an Agency Partner Grant.
The Agency Partner Grant (APG) Program is a grant program for Dutchess County non-profit organizations with a 501(c)(3) designation. Funding is awarded to organizations on a competitive basis based on their ability to demonstrate they can fulfill an unmet community need.
