Spring is the time to conduct transition planning from high school to adult services for your graduate. Similarly, spring is a time of transition at Cultivating Dreams. Flowers are blooming and our beautiful 10-acre farm is bursting with activity as we plant our seedlings in our all-season greenhouse. In the coming weeks, those seedlings will grow and be transitioned into carefully prepared raised beds. Just as those seedlings will outgrow the greenhouse, your student will continue to develop through new experiences. The Cultivating Dreams experience goes from seed to harvest and beyond; all enjoy a true farm to table experience.
What to include in a Transition Plan
Now is the time for transition planning from high school for students with disabilities. They are poised to begin the exciting next chapter of their lives. If your student has an IEP, it should include a strong transition plan. For example, the plan should describe your student's strengths, interests, and measurable goals. Does your student wish to continue education, work, or live independently? The plan should include services to help your student achieve those goals. Community Based Services (CBS) offers individualized, person-centered programs designed to meet the goals of the people with intellectual and developmental disabilities. Cultivating Dreams is one of CBS’s most innovative programs. Therefore, it can be a valuable part of your family member’s individualized transition plan (ITP).

Prevocational and Life Skills
Cultivating Dreams offers a transition from the high school classroom to an outdoor hands-on experience in gardening and small animal care/therapy. All participants partake in life skills, art programs, and physical fitness.
Nurturing Dreams
Just as we nurture the plants and animals on the farm, we nurture the dreams of the people we support. This is the cornerstone of the farm program. Our great passion is understanding those we support and how we can help them reach their goals.
